Der Osteopath Dr. Oliver Thomson ist außerordentlicher Professor am University College of Osteopathy in London und arbeitet als Forscher mit Schwerpunkt auf qualitativen Methoden. Mit seinem Podcast “Words Matter” erforscht er derzeit philosophische Grundlagen und die Rolle qualitativer Methoden in der manuellen Therapie.

WELCHES POTENZIAL SEHEN SIE IN DER PSYCHOLOGIE FÜR DIE OSTEOPATHIE?

Ich sehe den Unterschied zwischen Osteopathie und Psychologie nicht. In der Osteopathie geht es um die Pflege von Menschen durch Menschen. Der Akt der Pflege der Gesundheit eines anderen Menschen ist an sich ein psychologischer, sozialer und menschlicher Akt. Aus diesem Grund kommt die Ausübung der Osteopathie nicht um eine psychologische Haltung herum. Ob sich die/der Behandler:in dieser Haltung bewusst ist oder nicht, ist eine andere Frage. Natürlich kann die Osteopathie (und können Osteopath:innen) sich besser informieren und sich psychologischer Theorien und Erkenntnisse bewusst werden, um ihre Behandlung von Menschen weiter zu entwickeln und zu verbessern.

WAS WAR UND IST IHRE MOTIVATION FÜR DEN AUFBAU IHRES WORDS MATTER-PODCASTS?

Es begann, als die Pandemie zuschlug und meine klinische Praxis geschlossen werden musste. Ich wollte weiterhin produktiv sein und begann mit dem Podcast, der sich ursprünglich auf Kommunikation und Rückenschmerzen konzentrierte. Inzwischen hat er sich zu einer Reihe von Themen im Zusammenhang mit dem Gesundheitswesen entwickelt, von Philosophie über qualitative Forschung bis hin zum Wesen der klinischen Praxis. Ich genieße die langen Gespräche mit intelligenten Menschen… wir tauchen in Kaninchenlöcher ein und finden manchmal nicht mehr heraus. Die Gespräche zeigen hoffentlich, dass es selten eindeutige Antworten auf Dinge gibt und dass Kontext und Nuancen entscheidend sind… Dinge, die in den aktuellen Schlagzeilen und O-Tönen der sozialen Medien oft übersehen werden.

WAS KÖNNTEN EINIGE KLINISCH RELEVANTE ERGEBNISSE IHRER FORSCHUNG FÜR DIE OSTEOPATHIE SEIN?

Derzeit bin ich an mehreren Forschungsprojekten beteiligt, darunter eine nationale Studie mit gemischten Methoden über die Nutzung des biopsychosozialen Modells durch Osteopath:innen in Neuseeland. Eine andere Studie befasst sich mit den Fähigkeiten und Einstellungen von Osteopath:innen in Bezug auf die evidenzbasierte Praxis, wozu wir derzeit Umfragen in Italien und Kanada durchführen. Ein wirklich spannendes internationales Projekt, das ich leite, ist die Entwicklung und Erprobung eines Fragebogens, mit dem wir Daten über die therapeutischen Ansätze von Osteopath:innen sammeln wollen. Der Osteopaths Therapeutic Approaches Questionnaire (Osteo-TAQ) basiert auf einer Grundlagentheorie, die ich im Rahmen meiner Doktorarbeit entwickelt habe, und soll mehr Details darüber liefern, wie Osteopath:innen denken, wie sie mit ihren Patient:innen umgehen und wie sie mit ihnen interagieren. Wir setzen die Studie in Australien und im Vereinigten Königreich fort und hoffen, mit internationalen Kolleg:innen zusammenzuarbeiten, um das Instrument zu testen und zu übersetzen, damit es in verschiedenen Ländern und kulturellen Kontexten eingesetzt werden kann. Man hofft, dass das Osteo-TAQ in Verbindung mit den von den Patient:innen gemeldeten Ergebnissen und anderen Messungen eine bessere Vorstellung davon vermitteln kann, wie die große Bandbreite der osteopathischen Ansätze die klinischen Ergebnisse beeinflusst. Bei all diesen Projekten hoffe ich, dass das neu gewonnene Wissen die Osteopathie und die Osteopath:innen dabei unterstützt, ihr Denken und ihre Praxis kritisch zu reflektieren, um eine stärker auf den Menschen ausgerichtete und evidenzbasierte Art der Pflege zu ermöglichen.

Meine Erfahrung im Gespräch mit Philosophen und akademischen Kliniker:innen über die Anwendung von Theorie und Philosophie in unserer Praxis und Forschung. Die Osteopathie nimmt für sich in Anspruch, über eine starke theoretische und philosophische Grundlage zu verfügen; viele dieser Ideen wurden jedoch zu einer Zeit entwickelt, als Dualismus und Biomedizinismus vorherrschend waren.  Ich hoffe, dass die Teilnehmer:innen ein Gefühl dafür bekommen, wie Theorien von außerhalb der Osteopathie unsere Praxis prägen und unsere Forschung bereichern können.

The Osteopath Dr Oliver Thomson is Associate Professor at the University College of Osteopathy in London and working as a researcher focusing on qualitative methods. He is currently exploring philosophical underpinnings and the role of qualitative methods in manual therapies with his Words Matter podcast.

WHAT POTENTIAL DO YOU SEE IN PSYCHOLOGY FOR OSTEOPATHY?

I don’t see the distinction between osteopathy and psychology. Osteopathy involves the care of people by people. The act of caring for another person’s health is in itself a psychological, social and human act. For that reason the practice of osteopathy cannot escape a psychological stance, whether or not the practitioner is aware of this stance is another matter. Of course, osteopathy (and osteopaths) can become more informed and aware of psychological theories and evidence further develop and enhance their care of people.

WHAT WAS AND IS YOUR MOTIVATION FOR BUILDING UP YOUR WORDS MATTER PODCAST?

It started when the pandemic struck and my clinical practice had to shut down. I wanted to keep being productive, so started the podcast originally focussing on communication and back pain but it has since developed to explore a range of topics related to healthcare from philosophy, qualitative research, and the nature of clinical practice. I really enjoy the long-form conversations with smart people…diving into rabbit holes and sometimes never find our way our. The conversations hopefully show that there are rarely clear-cut answers to things and that context and nuance is crucial…things which are often missed by current social media headlines and soundbites.

WHAT COULD BE SOME CLINICAL RELEVANT OUTCOMES FOR OSTEOPATHS FROM YOUR RESEARCH?

I am currently involved in several research projects including a national mixed-methods study on osteopaths’ utilisation of the biopsychosocial model in New Zealand. Another study is looking at the skills and attitudes of osteopaths in relation to evidence-based practice, where we are currently running surveys in Italy and Canada. A really exciting international project is one that I’m leading whereby we are developing and testing a questionnaire tool which looks to gather data about osteopaths therapeutic approaches. The Osteopaths Therapeutic Approaches Questionnaire (Osteo-TAQ) is based on a grounded theory I developed for my PhD research, and looks to give much more details about how osteopaths think, relate and interact with their patients. We are continuing the study in Australia and the UK, and hope to work with international colleagues to test and translate the tool to be used in different countries and cultural contests. It is hoped that the Osteo-TAQ when combined with patient-reported outcomes and other measurments, may give us a better idea of how the vast array of osteopathic approaches influence clinical outcomes. With all these projects, I hope that the new knowledge which Is generate supports osteopathy and osteopaths to critically reflect on their thinking and practice, to provide more person-focused and evidence-informed ways of caring for people.

My experience of talking with philosophers and academic-clinicians about using theory and philosophy in our practice and research. Osteopathy claims to have a strong theoretical and philosophical basis; however much of these ideas were developed at a time when dualism and biomedicalism was dominant.  I hope that participants get a sense of how theories from outside of osteopathy can help shape our practice and enrichen our research.
